  2. Print out the form created at the next page and sign it in ink
  3. Mail it back to us at:

We would inform you by e-mail of the commencement date for the deduction as soon as your application is approved by your bank. On an average, it take about two months to process. In the meantime, please continue to pay your monthly subscription fee by cheque or cash until we receive approval from your bank.
